Stem Cells For Osteoarthritis: I Look Ten Years Younger Since my Facelift… But my Knees Still Feel Their Age… What Can I Do?

One of the first articles to mention stem cell therapy for osteoarthritis was an article published in the December 2003 article Arthritis and Rheumatism (Murphy JM, et al. Arthritis Rheum. 2003; 48: 3464-3474.)

The research team took two groups of goats and created osteoarthritis in their knees by removing the medial meniscus and cutting out the anterior cruciate ligament. After six weeks, one group of goats received an injection of sodium hyaluronan (a lubricant commonly used to treat symptomatic osteoarthritis of the knee) while the other group received a single dose of 10 million stem cells, originally extracted from the goats’ bone marrow, and then suspended in sodium hyaluronan before injection into the knee.

In the knees which had been treated by stem cell injections, researchers observed a regeneration of joint tissue and a decrease in cartilage degeneration.

The authors commented on their findings… "there may be a therapeutic benefit associated with local delivery of stem cells following traumatic injury to the knee. The longer term effect of this may be a reduction or delay in the progression to osteoarthritis."

The authors concluded that “this is a scaffold-free method for cell delivery and is therefore unencumbered by the complexities associated with placement of a solid cell construct.” This latter statement is important because so many techniques to date have involved the complicated use of different scaffolds (frameworks) to hold stem cells in place.

Stem cells are progenitor cells- the cells that all others are derived from. They have three interesting characteristics that make them prime candidates for tissue regenerative techniques. They are capable of dividing and renewing their numbers for an unusually long period of time; they are undifferentiated, meaning they have not yet committed to being a “heart” cell or a “lung” cell; and they can become any type of specialized cell. Another term to refer to this property is that stem cells are “pluripotential.”

While most research and ethical concerns have been centered on the use of embryonic stem cells, adult stem cells are more accessible with no ethical “baggage.” The cells that were used in the above study were adult stem cells obtained from the bone marrow.

At the Arthritis and Osteoporosis Center of Maryland, research efforts have been aimed at using adult stem cells obtained from the bone marrow to treat osteoarthritis of the knee or hip. Stem cells are obtained from the iliac crest of the patient’s hip and then concentrated using a special technique. The area of interest- usually the medial compartment of the knee is then prepared with a special instrument under ultrasound guidance. This technique ensures that local inflammation is created at the site of needed repair. This is a critical component of the procedure since the exact location of pathology will determine where the stem cells will be placed.

Along with stem cells, platelet rich plasma, also obtained from the patients is infused. The platelet rich plasma attaches to tyrosine kinase receptors on the cell surface of the stem cells and triggers gene expression within the nucleus of the stem cell. This “turns on” the stem cell to divide and differentiate into cartilage.

The entire procedure is done using local anesthetic with ultrasound guidance.

According to Dr. Nathan Wei, “Preliminary results are very promising. Longer term data is required and we are specifically addressing not only patient function but also measurements of cartilage thickness.” He adds, “the best approach as far as framework preparation for the stem cells and ensuring an adequate number of stem cells are of key importance. If you don’t have enough viable stem cells, the procedure won’t be effective. Also, the stem cells need to be primed properly… and have a scaffold (framework) to attach to. ”

Facelift Alternatives, A Complete Guide

Lately I've noticed a stubborn little crease on my forehead. I lift the skin just a bit, mimicking a facelift - and the crease disappears. During a facelift, the surgeon makes an incision, usually around the hairline, and then pulls the facial skin upward to smooth loose skin and wrinkles, and provide a more youthful appearance.

While a facelift can provide dramatic results, I'm not quite ready for all that. I'd just like something to freshen my face a bit. And yet I'm smart enough to know that some products and procedures billed as facelift alternatives are likely to have more of an effect on my wallet than on my face. But how to separate the facts from the fictions? I turned to an expert for help - and here's what I learned.

Topical Treatments

Dr. Joel Schlessinger, MD, FAAD, FAACS, president of the American Society of Cosmetic Dermatology and Aesthetic Surgery, assured me that creams can be very effective when targeting specific problems. No single cream can do it all, however, and sadly he could not tell me of a cream that is truly "better than Botox." He recommends the Obagi line for pigmentation and age spots, and he has found creams that accomplish such things as increasing eyelash growth and enhancing pouty lips.

Peels are another option, and their level of invasiveness depends on what you'd like. Prices range from low strength AHA peels at $30 - $100 per treatment to very strong phenol peels at $4000 or more. Chemical peels burn off the outer, damaged layers of skin, revealing the younger skin underneath. Babies have such soft skin because their cells are constantly regenerating, but this slows as we age. Chemical peels speed the process up.

There are some nasty stories about what happens if a chemical peel goes too deep, so be careful in your choice of practitioner. Some of the newer options, like blue peels, claim to be safer. At $200 - $1000, these peels may be adjusted to the desired strength. Milder peels can be repeated at regular intervals to achieve a desired result and minimize side effects and healing time. According to the blue peel website, a measured application of blue glycerine is mixed into the traditional peel chemical, trichloracetic acid (TCA), to "improve patient safety by visually indicating the TCA solution strength and facial skin coverage." Anything that helps keep the peel from going too far sounds good to me.

High Tech Salon Treatments

Laser treatments are increasingly popular these days, whether you're trying to remove unwanted hair or get rid of wrinkles. Laser "facelifts" use a carbon dioxide laser to remove outer layers of old skin and tighten muscles. Dr. Schlessinger tells me that the first laser treatments had "a long recovery time and fabulous results. Today's technology produces only about 20-30% of the those early results, but with almost no side effects." The new buzz word, Schlessinger tells me, is "fraxellation." In today's treatments, you can "strafe" an area so that the laser only disrupts pinpoints of skin, leaving enough surface untreated to speed healing time. This one sounded good to me.

What's more, laser treatments are only one kind of skin resurfacing. Dermabrasion and microdermabrasion, which can be thought of as extreme exfoliation, are old standbys that are still quite popular.

In dermabrasion, the surgeon removes the outer layers of skin with an electronically operated wire brush or diamond-impregnated burr. It can leave you pretty raw and is moderately painful, with a 2 to 12 week recovery. Microdermabrasion is a bit easier on the patient. It involves spraying fine crystals onto the skin, then removing them along with the top layer of skin cells. It is not as painful as dermabrasion, and is safe to repeat every couple months.

Microcurrent "facelifts" go deeper, using mild electrical stimulation to activate chemicals in the skin. Dr. Schlessinger warns that some microcurrent treatments are nothing more than quackery. In general, he says, be careful of spa-type clinics that recommend the same treatment for everyone-chances are they might own only one piece of equipment! Every woman trying to decide which procedure to have should have a consultation with a certified surgeon, Dr. Schlessinger tells me: "This is one area in which you get what you pay for."

Needles Required

Dr. Schlessinger cautioned me to be especially careful in selecting a good practioner if I decide to go with Botox. These days, Botox is being offered everywhere, from salons to parties. And yet, Botox is a powerful chemical that should be administered only by those with the training to do so. It is derived from a toxin that causes food poisoning. Injected in small amounts into the skin, however, it weakens or paralyzes certain muscles or blocks certain nerves, causing fewer frown lines and wrinkles.

"Many wrinkles are due to repetitive motion on the face-frown acquired as a habit," Schlessinger explains. These wrinkles disappear after a Botox treatment, and the very good news is that often after several treatments the habit is broken and no further injections are necessary.

But what about all the horror stories? The paralysis? Most of those are just urban legends. Botox is not permanent. In fact, injections last only three to four months, which means any side effects are also temporary.

According to Schlessinger, "the biggest complication we see is Botox done wrong." For two reasons, he advises going to a professional who specializes in Botox. One: the proliferation of Medspas and day spas has resulted in many unqualified practitioners. Two: you're more likely to get fresh Botox at a place that uses a lot of it. According to Dr. Schlessinger, we should buy our Botox fresh - just like we would want to get our fish fresh from a fish market.

Facial fillers are another injectable option. These injections actually help replace the volume that you begin to lose in your 30s. And sometimes they actually spur more collagen growth. A recent University of Michigan study even found that the supposedly non-permanent filler restylane sometimes provides long-term effects.

One popular name-brand filler is Juvederm, a hyaluronic acid dermal filler. Hyaluronic acid naturally occurs in all living organisms as a type of sugar. While the treatment does not last forever, it does last longer than Botox, typically at least six months or more.

I was also intrigued by the threadlift. In this procedure, barbed prolene facial sutures (threads) are attached to anchor points in the face (such as a sagging jowl) using a hollow bore needle. The threads are used to lift the sagging areas and can be tightened when necessary. I thought this sounded pretty cool, but unfortunately, it turns out that it's a risky procedure that Dr. Schlessinger says is "so popular, but so incredibly wrong." The threads used in the procedure are quite fragile and can break. When this happens, the effect becomes uneven and the threads can migrate to new areas, or even poke out through the skin! I have moved this option to the "no" column for me.

Go-Go Gadget Facelift

For those willing to try any weird science on their faces, there is such a thing as facelift tape, which uses exterior anchors and threads to physically pull the skin on your face tighter. Think Robin Williams in Mrs. Doubtfire. Unfortunately, you won't have a makeup team to help attach the device to your face every morning, and it seems like you'd need an awful lot of hair to hide the threads adequately. Not for me!

Sweatin' to the Oldies?

An au natural option is facial exercises. Rather than working from the outside-in, these programs claim to work from the inside-out, building tone and strength in your facial muscles. Some programs, such as Facial Flex, even include a resistance device that fits in your mouth to help improve the results of your "workout."

Sorry to say, Dr. Schlessinger says that these program claims are "a complete fabrication. If it were true, a person who chews a pack of gum a day would look great, but in fact the opposite is true." The more you stretch the skin out on your face, the more wrinkles you get. Remember, Botox works by temporarily paralyzing muscle groups, not by giving them a workout. I was a bit disappointed by this news, but at least I can take comfort in knowing there's something good about being a couch potato.

Minor Surgeries

While it may not seem like an alternative to some, there is the partial facelift option, in which a surgeon does not complete an entire facelift (which is extremely painful, expensive, and requires a large recovery time), but simply targets the areas you would most like to see improved. Neck lifts, upper brow lifts, and cheek lifts are common options, but the possibilities are truly endless. It's especially popular for women to have the fat removed under the chin, and in some cases, this is a liposuction procedure that doesn't even involve cutting removing skin. And it is, in my opinion, one way to look a lot younger through a relatively minor procedure.

Welcome to the New Millennium

It used to be unheard of to visit a plastic surgeon before you really needed it-at which point your skin and facial muscles are almost beyond repair. "Today," Dr. Schlessinger assured me, "we have better alternatives to keep people looking good for much longer." The trick is to do your research and take care of your face by starting early. And while an effective Botox cream is still five years off, new treatments are developed every day. With so many good options, short of a full facelift, I am very happy to know that I will never be trapped in a prematurely aging face.

Do It Yourself Non-Surgical Facelift

I’m the first to investigate a new anti-aging product or procedure, and have been for the last twenty years. Sometimes I’ve even gone so far as to indulge myself with the promise of eternal youth. So either the earth’s gravitational pull is getting stronger or raising children has finally taken its toll. Here I am, middle aged and wondering what’s the next thing to “go south.”

I’m an esthetician with all the tools of the trade. I have all the gizmos and gadgets that when used regularly will minimize the effects of aging. So why don’t I use them on myself? Because it seems that there is never enough time.

With all the new technology such as lasers, thermage, microcurrent and magic lotions and potions, there is always a cost. It might be the price, the risks or the inconvenience. What if there was something you could do in the privacy of your own home for as little as 10 minutes a day. I know, I’m beginning to sound like an informercial but the claims are that there really are things you can do that can help get that youthful jawline and line free forehead back. Would you be willing to give 10 minutes a day for a few weeks to try it and see for yourself?

Facial exercises have been around for ages. Now it makes sense that toned muscles are firm muscles and that goes for the face as well as the body. As we age the underlying muscles of the face become flaccid and are less able to support the skin that is attached to them. While there is a lot that can be done to tighten and firm the skin, it is the underlying muscles, that when toned and tightened, make the biggest difference.

My first experience with facial exercise was with Greer Childers in her exercise program, “Body Flex.” She based her technique of the premise that oxygenation of the tissue keeps it young and vibrant. She had me doing the “Lion” and the “Ugly Face” (appropriately named) where I would get into position and pull these horrible grotesque faces and hope to high heaven no one walked in on me. She promised me that while everyone else was trying to “smooth the wrinkles out of their face we will have the last laugh because we will already be there.”

Then came Ellae Elinwood with her book, “Timeless Face.” This is a book I got not only for myself, but for my clients to prolong the effects of their facial treatment. Ellae has twenty three facial exercises that you do in front of the mirror....alone... or with a group of five year olds for entertainment.

Another favorite is “Facelift at you Fingertips,” by Pierre Jean Cousin. I really like this book and incorporate a lot of the techniques in my facials. This book focuses primarily on acupressure points, massage and aromatherapy. By stimulating acupressure points in the face by massage or gentle tapping, you increase the flow of energy, increase circulation and improve muscle tone.

Now I don’t know about the facial gadgets you put in your mouth to help exercise and tone the muscles. Besides the sanitation issue, I question the effectiveness. The only thing I will put repeatedly in my mouth is my tooth brush.

One company sells a workout ball and a towel to heat and warm up the face before exercise. I guess this is so you don’t pull an orbicular oris muscle during your workout.

The most important exercise you can do to improve the face is to smile. Have you ever noticed how people who smile more tend to have a definite jaw line. If jowls are your problem try looking in the mirror and then smile and watch how the jowls disappear. Smiling can relieve stress and tension and help you feel better. Your smile will have an affect on others when they see you smiling. You can even use surgical tape while you sleep to keep that smile in place.

Incorporate these facial exercises into your daily exercise routine and you can be firm and toned head to toe.

A Weekend Facelift

Facelift or Rhitydectomy is a popular cosmetic surgery where the skin on the face and neck is pulled back to remove wrinkles, sags, jowls etc to give a firm and youthful appearance to the face. As a person ages, the skin on the face gets wrinkled, the jaw line grows slack and jowly. This is due to gravity, sunlight, wear and tear and stress on the facial skin.

In a facelift operation, a local anesthesia or a general anesthesia is given to the patient. Incision starts from the ear to behind the ear and ends near the hairline behind the ear. After the incision, undermining is done. Undermining is separating the facial skin from the underlying tissue below the neck, chin and cheeks using scalpel and scissors.

After undermining the facial suspension system are tightened with stitches. Next the skin is pulled backwards and upwards to the required extent. Then the skin is tightened with stitches. The underlying excess tissue may or may not be removed. The excess skin is removed and the incisions are closed with sutures.

Generally the incision is hidden around the ear and extends to the earlobes and into the hairline. Hence the incision marks are inconspicuous. For a facelift operation a thin skin and good bone structure is ideal. After the facelift operation a windswept look to the face is common. In men the side burns turn backwards giving an unnatural appearance. In women the earlobe is pulled downwards. Facelifts are generally combined with other operations like eye surgery (blepharoplasty) and skin tissue resurfacing with lasers.

Before undergoing facelift operation, the patients are advised to stop smoking and drinking at least a month in advance. Growing longer hair is better to hide the incision scars. Recovery takes up to a month but most patients generally goes back home within a day. Sometimes tubes are attached under the skin to drain out excess fluid for a day or two.

There can be complications like bleeding, damage to facial nerve, necrosis (death of skin), hair loss near the incision. The risk of post-operation complications is higher in case of patients with diabetes, hypertension, smokers etc. However they are not common.

Patients should not take aspirin or anticoagulants before surgery. Further patients should discuss all their medical history and whether they are under any medication with the plastic surgeon.

In the US it is desirable to be treated by a surgeon who belongs to association of plastic surgeon (AAPS) or any other national association. However all qualified surgeons are authorized to make a facelift operation. Results vary from subtle to dramatic depending upon the appearance of the patient before operation.

This growth period normally occurs from ages 14 to 15 for girls and somewhat later for boys and your nose is generally fully developed by the age of 16. Once you are done with the growth spurt of puberty - which is when you stop getting taller - it is fine to have a rhinoplasty.

Rhinoplasty can be performed as a day case, which means that you can have the operation and go home the same day. Pain, bruises and some swellings are expected after the procedure and there might be some minor bleeding for a day or two. Alloplastic synthetic materials are sometimes associated with long-term complications such as migration and extrusion. The swelling, which will reside mostly around the eyes and the nose itself, will be very noticeable at first, but will begin to subside around the third day and should be largely gone within two weeks. Some minor swelling will remain for up to six months afterwards, and the final, long-term rhinoplasty results can be evaluated and enjoyed about a year after the surgery.

It is not recommended for extensive reconstruction or surgeries that require a lot of work done on the cartilage at the tip of the nose. The two biggest advantages to closed nose jobs are the lack of any scars, and usually, a shorter surgery time.

Your image immediately following surgery will be startling. You will be bruised and swollen after it has been completed. You probably won't like what you see in the mirror. Your nose might be packed with gauze. You will have a cast on your nose to support the healing nasal structure. Your doctor will tell you whether or not to keep ice or cold compresses on your nose, some people feel that bags of frozen peas make a good ice pack, as they mold to your face.

Your nose will be stuffed up, and you'll have to breathe through your mouth. You'll also feel some pain from the procedure, and you may become nauseous or throw up. Tummy Tuck Surgery Without Drains

Tummy tuck or abdominoplasty surgery still remains as one of the most dramatic and satisfying of the many body plastic surgery procedures that are available. Removing loose skin and fat, tightening the rectus muscles, and narrowing the side of the waistline with liposuction always makes for a significant change in one's waistline. Even when done in a more limited version (mini-tummy tuck), the flattening effect is always appreciated. Despite these improvements, the one aspect of tummy tuck surgery that patients, and I, did not like was the need for postoperative wound drainage.

In the past, tummy tuck surgery (abdominoplasty) always required the use of drains after surgery. Staying in place for up to two weeks after the procedure, they were necessary to remove fluids that accumulated from the open space or layer left behind. (between the underside of the skin and the muscle)They could not be removed until enough internal healing had occurred as evidenced by a decrease in the amount of drainage that came out of the tubes. By the time this happened, many patients had a fair amount of soreness from the skin areas where the drains exited. In some cases, even though drains were used and removed when their output was low, patients still developed fluid collections weeks later that required multiple in-office drainage procedures.

As an improvement to the tummy tuck procedure and to eliminate their inconvenience and discomfort, I have adopted an innovation that makes the use of drains unnecessary in many abdominal contouring procedures. This is a technique known as 'quilting' or partially closing off the space in which fluids can accumulate. Using special sutures that tie-down the underside of the skin to the muscle, the typical 'dead space' left behind is closed down. Dead space refers to the unhealed open area or layer between the muscle and the overlying skin which is created from the way that a tummy tuck has to be done. With little space for fluids to accumulate, the need for drains after surgery can be eliminated in many cases. This does add some extra surgery time, but to any patient who has ever had drains, that extra time would be viewed as a good investment!

While not useful for every tummy tuck (really big ones still need drains), the majority of cosmetic abdominal contouring procedures can now be done without drains. Even if a fluid collection or seroma does occur, it is much smaller and needs less in-office drainage procedures later.

Silicone Breast Implant Problems - What's Old is NOT New Again

Now that silicone gel breast implants have been officially back and FDA-approved since 2006, most breast augmentation patients are opting for it. While it is more costly than saline breast implants, silicone breast implants offer a more natural feel, have very little to no implant rippling, and will not deflate in size if they rupture. While most patients prefer silicone over saline, and do eventually opt for it most of the time, there still remains concerns about their historic safety. Even very young patients, who were just young kids at the time of their removal from the market in 1992, will ask about these now near ancient breast implant issues. In listening to what their concerns are, it is apparent and understandable that patients confuse local complications with systemic complications when it comes to silicone breast implants.

Local breast implant complications exist today as well as decades ago....and they are the same whether they are saline or silicone breast implants. Local complications refer to 'hardening of the breast implant' or the formal plastic surgery term, capsular contracture. The formation of scar around any implant in the body is normal and is how the body reacts (favorably) to a foreign body that it 'tolerates'. This scar is usually very thin and is undetectable by feel, making the breast soft and easily moveable. For reasons that are usually unknown (the leaking of silicone material through or out of an implant is a known cause), the scar around a breast implant may overreact and get quite thick. When this happens to a circular scar around a sphere (breast implant), it can start to contract making the breast hard, immobile, and even distorted if severe enough. This is known as capsular contracture and was quite common with old style silicone breast implants, particularly when they were placed above the muscle which was the norm in the 1980s and early 1990s. While troubling, capsular contracture does not have any harmful effects on the body other than the feel and shape of the breast itself. It is a local complication. The occurrence of capsular contracture in breast implants has been dramatically reduced over the past 15 years as the implants are now routinuely placed under the muscle rather than on top of the muscle. Why this works in not well understood but the combination of constant massage from muscle movement and that the tissue surrounding most of the implant is muscle (very vascular) rather than breast tissue are obvious reasons.

Systemic complications from breast implants refer to the potential for creating sickness of the body, specifically autoimmune diseases. While once touted as 'fact' by many, careful scientific scrutiny has failed to establish any such link. The old style silicone breast implants have been studied more than any medical implant in the body in the history of medicine and the causation of systemic health risks from them has been debunked. Today's silicone breast implants are better made and the leeching of any silicone particles is very low compared to implants of old. It is for this reason that the FDA has released them back for commercial use. Patients should not fear today that any such systemic health risks exist. Yes, there will always be a few unfortunate patients who will develop automimmune diseases and other health issues that have breast implants but this association is coincidental, not cause and effect related.

Silicone breast implants, like saline breast implants, are health-wise safe. They have the potential for local complications over one's lifetime, such as capsular contracture, but the occurrence of this problem is fortunately low and capable of being successfully treated.

Breast implants can be positioned either in front or behind the chest muscles. Once again, there are advantages and disadvantages to each.

Subglandular implants are positioned behind the breast but in front of the muscle. This involves simpler surgery and a quicker recovery, however the risk of problems with the implants slightly increase. Such implants can also interfere with mammograms.
Submuscular implants are placed behind the chest muscle wall. This involves more complex surgery with a longer, more painful recovery time, however these implants are less problematic and do not affect mammography.

Procedure

The route taken into the breast will affect scarring and future function. There are three commonly used access routes:

Periareolar - where access is gained via a small incision around the nipple. This leaves little noticeable scar but may impair nipple function and prevent breastfeeding.
Inframammory - where access is gained from a small incision directly under the breast. This does leave a small scar but does not affect breastfeeding.
Axillary - used less frequently than the other routes, involving a small incision near the armpit. This does not impair breastfeeding.

Naturally, your surgeon will advise you on all of these issues to ensure you get the treatment that is right for you.

Recovery

In some cases, breast implants can be done as a day case, and you will not have to stay in hospital. In most cases, however, you should expect to stay in for one or two nights.

After your surgery, your breasts will feel tender and sore, and you will need to take a week or more off work to recover. If you have any concerns during this time, such as unusual swelling, soreness or pain, or an intense feeling of heat in the breast, you should contact your surgeon immediately.

The initial swelling and bruising will quickly fade; however your new breasts may feel tight for some time, as it takes several months for the skin, tissue, and muscle to grow and stretch around the implants.

Risks

Modern breast implants are very safe and ruptures are now rare, however they are still possible. If this happens, you will need to undergo further surgery to remove the ruptured implant and replace it. Despite the advances in design, it's still rare for a woman to go through her whole life with the same implants and you should be prepared to have them replaced or removed at some time in the future.

Many people have written me about the safety of silicone gel breast implants and how to decide which type of implant to choose. I am reprinting here one response which I think presents helpful guidelines for women considering breast augmentation or enlargement, or Breast Lift, and trying to decide which breast implant to choose, saline or silicone gel.

There are many factors to think about when deciding whether to have breast enlargement or augmentation. The newer silicone gel breast implants have been approved by the FDA and have been available since January 2007. Many women share your concern about the safety of these new implants for breast augmentation.

My first comment is that breast augmentation or enlargement, if done on an appropriate candidate by an experienced board certified plastic surgeon should produce an equally pleasing and successful aesthetic or cosmetic result with either type of implant.

The main difference in the result of breast augmentation with saline breast implants from that with silicone gel breast implants is the feel of the implant. If there are no complications such as hardening of the implant, and the result of the surgery is satisfactory, then a silicone gel implant will feel more natural than a saline implant.

There are many factors which contribute to the different feel of the Saline implant and most of them can be predicted or managed. In very thin women with very little breast tissue to cover the implant, the saline will be more palpable. If a capsule forms around the saline implant, it will be more palpable. If the surgeon does not over fill the saline implant, palpable folds can develop.

Placing the saline breast implant beneath the chest muscle helps to prevent palpable folds in the implant and over filling the implant helps prevent folds. Meticulous surgical technique and avoidance of bleeding during surgery also helps avoid the capsule which is scar tissue which makes the implant hard.

A silicone gel breast implant which has been placed under the breast or muscle, with meticulous technique and which does not form a capsule or hardening does result in the most natural feeling breast. A "perfect result" with a silicone gel implant does feel better than a "perfect result" with a saline implant, but visually, the way they look, is about the same, in my opinion.

There are however disadvantages to silicone gel implant breast augmentation or enlargement. The new FDA guidelines require that you have frequent MRI exams to check for rupture-this is expensive. The risk of capsular contraction of the breast implant is about the same with the newer gel implants as with the saline implant-13-15%. In addition if the implant does rupture and need to be replaced, the surgery to replace a silicone gel breast implant is more complex than that for a ruptured saline breast implant.

Most women in my practice find the decision an easy one. Many patients are very health conscious and only concerned with safety, want the least risk, don't want to bother with MRI's, and are most concerned with how the breast looks and don't care about a subtle difference in feel and want the least risk of firmness or capsular contraction of the breast implant. These patients are usually married, have children, and are in their late 20's to early 40's. They typically choose saline breast augmentation.

By far the largest group who select silicone gel breast augmentation are patients who have had a previous breast augmentation, usually with saline implants, and want a more natural feeling breast implant and choose to have replacement with silicone gel implants. The other large group of patients selecting sgel breast implants is younger women, single with no children, who have read a lot about breast augmentation and want the most natural feeling implant.

It comes down to your priorities and your situation. Silicone gel breast implants are safe and if there are no complications, they produce a beautiful natural feeling result.

If you want the peace of mind of not worrying about firmness and having to have MRI's and worrying about the possibility of rupture and you are willing to accept the fact that the implant will be subtly palpable and MAY (many do not form folds) form palpable folds that likely only you will notice, the saline breast implants are a better choice for you.

If you want the best feeling or most natural feeling implant then silicone gel breast implant is the best choice.

The most important thing is for you to find an experienced board certified plastic surgeon who has done many breast augmentation operations and can give you an honest opinion.

In my opinion the most important factor in achieving a good result with either type of breast implant is the skill and experience of the surgeon. This operation needs to be done meticulously and carefully, and not all surgeons take this approach.

Breast Implant Projection - How Does it Influence Final Breast Shape?

Breast implants are available not only in different sizes but each size has differing projections as well. Projection involves a change in the manufactured shape of the implant so that more volume can project forward at the expense of narrowing the implant's base diameter. A high projecting implant, therefore, has the theoretical possibility of making the final breast shape fuller on top than lesser projecting breast implants.

When considering breast augmentation, women are focused on two different types of final breast shape. The first is the more natural tear-drop shaped breast that has more fullness in the bottom of the breast than the top as seen in a side view. The other is a rounder shape (some call it unnatural) than is fuller on the top of the breast than the tear-drop shape and may even be equally as round on top as the bottom of the breast. Neither shape choice is right, it is a matter of personal preference.

Because different breast implant projections exist, it is often perceived that the manufactured shape of the implant will create the final shape. While there is some merit to this thought process, I think it is overstated and, in some cases, completely deceptive. My experience is that the amount of existing breast skin and the size of the implant chosen are far more important factors in creating one of these two final breast shapes than the implant's projection. For example, if one's breast skin is fairly tight and a large implant is chosen, it will look round no matter what implant projection is chosen. Conversely, a lot of loose breast skin with a moderately-sized implant will appear tear-drop shaped no matter what the implant's projection.

While implant projection is a useful feature, it is a minor determinant in the outcome of the final breast shape after augmentation. The consideration of the tightness or looseness of the breast skin and the chosen size of implant will override the projection feature.

Breast implants have been on the market and in use since 1962. They have been a popular resource for as many as 1.5 million to 2 million American in creating the perfect breast augmentation since that time. Various types have been available including silicone and saline filled devices. Back in 1992, however, after some circumstantial evidence linking silicone implants to connective tissue diseases and cancer, the Food and Drug Administration put a ban on the use of these devices in enlargement procedures.

Up until that point, 90 percent to 95 percent of augmentation patients had chosen silicone because they like the rounder, fuller appearance. In the decade that followed the percent flip-flopped with 90 percent to 95 percent of patients using saline-filled devices. The ban was lifted in November 2006, and since then women have had the option of either silicone or saline.

It is interesting to note that in the same year the FDA banned silicone implants, the National Cancer Institute initiated a thorough study researching the relationship between implants and cancer rates. The study included 13,500 women over the course of 13 years, on average. What the NCI researchers found is that there was definitely "no association between implants and the subsequent risk of breast cancer". This was true of silicone as well as saline implants.

Not only were breast augmentation patients not at greater risk of breast cancer, but they were not at any greater risk for cancer of the stomach, large intestine, cervix, uterus, ovary, bladder or thyroid. The study further found that women who had undergone enlargement did not have higher rates of connective tissue disorders or immune system cancers.

The NCI study did find however, that those with breast implants that ended up developing breast cancer had a slighter slower rate of detection than those without these devices. The mortality rates of cancer were no different, however, and women with implants can get around the slower rate of detection by having annual mammograms.

And if that study wasn't enough to assuage consumers' concerns about the safety of augmentation surgery, a new study published in January 2009 in the International Journal of Cancer confirmed the results of the NCI study. The new research was conducted among 6,200 Swedish and Danish women with breast implants and tracked them on average, 15 years after their initial surgery. The results were that the augmentation patients do not have a "higher-than-average risk of cancer years after having the surgery".

This is, of course, great news for those considering enhancement surgery. It means that women looking for breast enlargement do not have to worry about getting cancer from the small chance that their devicesmight leak or from their body's reaction to the implants.

This permits the implant to not be overly compressed during insertion, protecting the implant's shell and lessening the long-term likelihood of implant rupture.

While the natural lower breast crease is usually a good place to put an incision on the breast so that it is the least visible, its placement in one's existing crease may not always be best. In many breast augmentations, the bottom part of the breast must be expanded so that the a new and lower breast crease will actually be established after. Remember breast implants make the overall breast bigger in all dimensions and this includes the lower pole. For this reason, particularly in larger breast implants, the incision must be made lower than the existing lower crease so that it does on the breast itself after.

It is important to rest with your head elevated the first few days. This helps get rid of swelling. No heavy lifting or exercise the first week as these activities could result in post operative bleeding.

Most people are ready for eye makeup by day four, and many people are comfortable in public wearing sunglasses after a day or two.

Who Should I Consult for Eyelid Rejuvenation?

I recommend a board certified plastic surgeon that specializes in laser surgery and eyelid rejuvenation. The laser transconjunctival blepharoplasty with tissue preservation and fat grafting or repositioning is a fairly new procedure and not many plastic surgeons are familiar with this technique.

Even fewer plastic surgeons are using the laser technique.

You will have to do your homework. Search for laser transconjunctival blepharoplasty and fat grafting or fat repositioning.

Try to consult at least two surgeons and as to see photos of their results with the newer technique. Ask how many of these operations they have done and evaluate their response.

Modern transconjunctival and laser blepharoplasty techniques with tissue conservation are a vast improvement over techniques that were available just two to three years ago. Beautiful eyelid rejuvenation with quick recovery and normal eyelid shape post operatively is possible today. Bags under eyes, puffy eyes, eye hoods, eyelid wrinkles and crow's feet can be removed with a minimal down time and very little evidence that you have had surgery.
